Hot flashes are common. Suffering through them doe Hot flashes are common. Suffering through them doesn’t have to be.

Two-thirds of women in the U.S. experience hot flashes, often for years. They can show up as a sudden wave of heat in the middle of a meeting, on your commute, or while you’re trying to enjoy dinner with friends. The discomfort can be distracting, and the embarrassment in social or work settings is real. 

At night, they can leave you drenched, tossing off the covers, and waking repeatedly, exhausted the next day.

Estrogen therapy is the most effective treatment for hot flashes. But not everyone wants—or can—use it. That doesn’t mean you have no options. Breathwork practices, identifying and avoiding triggers like alcohol, hot drinks, or spicy food, and from an herbalist’s perspective, the wise use of botanicals, can all play a role.

While data are limited—largely because pharma has little incentive to study herbs—research and traditional wisdom show that botanicals can ease hot flashes, improve sleep, and lift mood. Limited research does not equal lack of efficacy. And traditional knowledge, carried forward by generations of women, is a form of evidence too.

Here’s a blend I’ve recommended in my practice for many years. You can purchase individual tinctures from companies like Mountain Rose Herbs, mix your own 4 oz. bottle, and even make a batch for a friend:
🌱 1 oz. Chaste tree
🌱 1 oz. Lemon balm (supports mood and sleep)
🌱 ½ oz. Motherwort
🌱 ½ oz. Black cohosh
🌱 ½ oz. Hops
🌱 ½ oz. Sage
To prepare: Combine in a 4 oz. dropper bottle.
Dose: Take 1 tsp in ¼ cup hot water twice daily. One dose may be taken before bed to support sleep and reduce night sweats.

⚠️ Not for use in 🫄or🤱. Omit sage and hops if you have a history of estrogen receptor–positive cancer.

Here’s the thing: in medicine, being labeled “ Here’s the thing: in medicine, being labeled “that patient” is real. 

Sometimes it’s said with an eye roll in the exam room. Sometimes it’s written into your chart. And once it’s there, it can follow you from one doctor to the next. It’s shorthand for: she asks too many questions, she pushes back, she won’t just go along.

But history shows us why this happens: women’s pain has been dismissed for centuries. We’ve been told we’re “hysterical,” “emotional,” or “noncompliant.” And so many of us have learned to censor ourselves to stay polite, apologetic, agreeable just to avoid the label.

Yet here’s the truth: asking questions, saying no, or trusting your gut doesn’t make you “difficult.” It makes you discerning. It makes you informed. It makes you powerful.

As a physician and midwife, I can tell you this with certainty: women who advocate for themselves not only change their own health outcomes, they change the culture of care for all of us.

Perimenopause doesn’t have to feel like a free fall.

In my practice (and in my own perimenopause experience)  I’ve seen how much the basics matter. 

These aren’t quick fixes or replacements for medical guidance including menopausal hormone support when needed, but are the cornerstones of true health and longevity. 

They’re the daily practices that regulate hormones, lower inflammation, and create resilience for the years ahead.

Here are the habits I return to again and again:

🌱 Adequate sleep — because circadian rhythm is one of the most powerful regulators of hormones.

🌱 Strength training — protecting muscle, metabolism, and bones into menopause and beyond.

🌱 Blood sugar balance + hydration — steady energy, fewer mood crashes, and long-term metabolic health. 

🌱 Alcohol (very little, or none). It disrupts sleep, worsens hot flashes, and drives inflammation.

🌱 Time in nature — one of the simplest ways to calm cortisol and restore nervous system balance.

🌱 Community + connection — girlfriends, laughter, belonging. Loneliness is one of the biggest predictors of poor health outcomes. Social connection, on the other hand, increases oxytocin, lowers stress, and literally protects longevity.

🌱 Keeping inflammation in check — the through-line for healthier cortisol, steadier hormones, and reduced risk of chronic disease.

Why this matters: The choices you make every day influence the very pathways that shape your hormone health.  Not just now, but for decades to come.
